referral process

FACE of Boone County welcomes referrals from private and public Boone County entities.

eligibility criteria

In order to be eligible for services through FACE, students must:

  • Be a resident of Boone County
  • Be between the ages of 0-19
  • Demonstrate a social, emotional, or behavioral health need

exclusion criteria

Youth meeting the following conditions may not be appropriate referrals for FACE if they:

  • Have an open Children’s Division case, not within 30-60 days of being terminated/closed or THP
  • Are successfully participating in case management, mental health, or behavioral health services
  • Are experiencing immediate suicidal or homicidal ideations
  • Are currently intoxicated or under the influence of a substance
  • Have emergency medical needs
  • Do not demonstrate a mental health or behavioral health need
  • Are in need of residential treatment

Making A Referral

With the permission of the family, anyone can make a referral to FACE. Examples of referring individuals include, but are not limited to:

  • Parents
  • Guidance/Outreach Counselor
  • Clergy
  • Problem Solving Team Member
  • School Social Worker
  • Boone County Schools Mental Health Coalition Regional Coordinator
  • Law Enforcement
  • Friends
